Normally I'd recommend Olimex in Bulgaria (even though you specifically
asked for the US).  But it sounds like you need someone who can/will
work from an art print - not gerbers.  I've used Circuit Design for such
a thing with good results (quality approx what you could do in your
garage w/photo etch *if* you had it all together).  Prices are very
resonable.  I tried their website but it was gone -  hope they're still
around.
