Sorry, no docs, but i've done it (well, an arp avatar), as well as had CMS
do one.

Things I'd do differently:

1) make sure everything's working cause it's harder to repair once it's
done (more things to unscrew)

2) absolutely check for clearance on jacks

3) get the hex nut minijacks, not the knurled (i misordered from mouser,
and the hex nuts are easier to tighten and look better)

4) iirc, i set mine like Phil C's so that patching comes after the select
switch.  In retrospect, it might be more useful to have it come _before_
the switch, so that you would retain half the functionality of the switch.

5) Phil C's prices are really good.  I had it done several years ago, but
it was only $4 or $500, and something like 35 holes.  That's maybe $15 /
hole / solder joint.  Really.

Having modded one, i'd considering doing it again (since I've already cut
my teeth), but it's hard to beat $15/hole to drill, snip, and solder.

p.s. the mod is great for fm synthesis.

p.p.s. One other mod I wish I'd taken Phil up on - replace the VCA.  That
thing is noisy!

p.p.p.s.  Each of the components (VCF, VCA, etc) are clearly boxed and
labeled on the schematic, including signal flow.
