No, you don't actually need any components to make a Soundblaster Live (or many other sound cards) work as a MIDI interface. The actual cable which you can buy is a simple connector, D on one end, 2 MIDI plugs on the other end. The pdf which Chris Randall posted has the info how to wire your own.
BTW, my Soundblaster Live also has a second backplate with digi-IO, and 2 little sockets for MIDI In+Out - the sockets are like PS/2 sockets (can't remember if they are exactly PS/2) and you can get little adapters which bring them out to full size 5pin DIN sockets. There are also many other SBLive variations, some of which have breakout panels which mount in a CD drive bay. I believe most of (all?) the extra I/O is present even on the cheap SBLive versions, so I would imagine you could also take MIDI straight off one of the jumpers on the board to make your own breakout panel.
