[sdiy] CA3018 AP Note Wanted.

Howdy,

A number of the RCA/Harris/Intersil transistor array data sheets mention an
app note: "AN5296 Application
of the CA3018 Integrated Circuit Transistor Array". It's not in my old RCA
data book, no longer on the Intersil WEB site and I wasn't able to find it
with Google. Would anyone happen to have it scanned in and be willing to
share?
