Most PCB places will no longer take photo artwork....the best thing to do 
is download a free PCB program from the Internet and do a new PC Board 
layout.  In the Denver area (well, at least Colorado), there is Advanced 
Circuits ( http://www.4pcb.com ).  They will make 3 boards for $99.  They 
do really nice work, but, like I said, you need to send them Gerber and NC 
Drill info to get that price.  You will probably pay about $300-$500 in NRE 
to just get a drill tape made for your board.
