The PAiA 9710 Voltage Controlled Mixer w/Modulator is a "VCA/Balanced
Modulator/Noise Source/Mixer/ADSR all in One":

The ADSR can be used as a LFO (self-triggering mode).
It has 2 VCAs and a built-in panning control input.
In other words, it's very close to what you want, and most likely cheaper
than a custom module.
You could also build something around 2 "dotcom" panners

or the Blacet quad VCA
http://www.blacet.com/VCA.html.
Those solutions would require additional modules, like an LFO and possibly
an inverter, whereas the PAiA module would do it all (just needs a power
supply).
I know you asked for a custom module, but this may be a good time to go
modular. Start with enough to make do panning, and you can build from there.
