I just looked at www.tubesandmore.com and found this one:

which should work fine. There's two others that should be OK if you 
spend some time making sure you avoid ground loops:

P-R4FB3A1B
P-R9FB2A1C

For these last two you might have to fiddle with the grounded input 
or grounded output jacks to remove the ground connection. Could be a 
hassle.
