Don't know any good ref's re: VST Programming.   But you should have no
problem getting The "DSP Filter's book" inexpensively as in is rather
pointed toward the companion code which was able to be downloaded from
SAMs prior to it's sale to another book firm which doesn't have the code
and isn't interested in finding it (i wondered why the book was so
cheap?). 

It is a good read, not really dissimilar to other Ref's with a good
discussion of specific techniques being implemented in a very general
way using C++ as a form of pseudo-code. 

Also Analog Devices was and may still is giving out "Digital Signal
Processing: A Practical Guide for Engineers and Scientists" which is
also sort of introductory but uses a form of "Basic" as pseudo-code for
the general algorithms explained and is pretty comprehensive while still
general.   Beside being sent the hard copy I downloaded a machine
