Elpac makes dual (+-12V) and triple (+-12V, +5V) both in wall-wart and 'table-top". Some have 'universal inputs' (90VAC-250VAC) to operate word-wide. Uses a 5-pin MIDI male connector for DC out. Not cheap (say $40-$80) but I've used them for years. Mosuer, Digikey and Newark carry them. Paul S.
