I've got a board called the cansynth which has some of these features.
8 10bit inputs and 8 8/10/12 bit outputs depending on which dac you solder in.
However, it needs external circuitry to interface with standard modular
voltage levels.

I've also written a colorforth kernel for the avr which I'm using to develop an
additive synth. It may be useful even if you use a different board (with an avr).
