It's such a marked jump that my first thought was that it is something
'switching on', so my suspicions lean toward the test set-up. Could it be a
substrate transistor turning on if the output swing is too large? (Don't
know that that would account for the different behaviour at different
frequencies, or the apparent hysteresis though.) It would be nice to see a
scope trace of the output when it happens, or at least know the magnitude of
the test signal applied as 'Vin' (didn't spot that in the document?).

Some interesting plots though.
