This is correct - the US model type was Yamaha PA-3B and the specs were:

Input: AC 120v   60Hz   15VA
Output:   DC 12v   700 mA
Polarity: inner + outer -

The PA-3B is very common to Yamaha PSR keyboards and can be found easily at a music 
retailer, home electronics store or online.
