I've had this happen as the result of bogus SysEx dumps. In every instance I've encountered either the .mid was corrupt or the data transfer botched up, i.e., using a USB MIDI Interface. These instances are very rare and it always clears-up with a power off/power on. Never happens under normal use or with AN1xEdit. Don't know that this is identical to what you describe.
