I had a similar problem and found that I had to click
the "program" button (ctrl-p) rather than hit the
"write" icon + On another occasion I had the device go
missing and rebooting my pc did nothing - I needed to
totally power it off to recover.

Don't know if you've tried this but it worked for me.
