The way atmel did the fuses is a bit confusing, and that may be what's going on. One thing to check that I didn't see mentioned, is that CKOPT needs to be changed. The default state enables a low power "vittoz mode" oscillator that may or may not run with a given crystal, or worse, will run just barely enough amplitude to clock most of the chip, most of the time.
