Two simple possibilities

1. Maybe something outside of the chip is causing the increased
voltage at the output of the chip. It would depend on what the output
impedance of the chip is and what it is connected to. I would suggest
you disconnect the outputs before measuring them.

2. Faulty IC, it doesnt happen often, but can still get you now and then.

Move to quarantaine

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
