It's a CL Hex Rod.  I recently got one for $10 shipped.

As for the clamp, I wouldn't waste money on another plastic clamp.  

You have a couple options.

The first is to buy a Gibraltar SC-GPRTC Power Series T Clamp.  It's
made entirely from metal and is the logical upgrade to the plastic
clamp.  About the cheapest I've found them is from
www.waddellsdrums.com .  They're $15.50 and shipped for free.

The second option is to use a Yamaha Tom Mount CL-940 with any of the
Gibraltar Multiclamps (Power Series SC-GPRMC, Road Series SC-GRSMC and
SC-GRSSMC).  The CL-940 I've seen for $17 at Drumbalaya, and the
multiclamps from $15.50 to $30.00 each.  Do a search and find the best
price, but Waddells Drums seems to be about the cheapest on these.
This combination of Yamaha Tom Mount and Gibraltar Multiclamp is very
similar to what is used on the DTXtreme kits.  It looks the best, is
more adjustable than any other setup, but costs the most.
