Digital BW, The Print

Interesting discussion on Book printing. 

Has anyone had any successful Print on Demand books done in Black and White? 

Does anyone know of any of these publishers that is using a press other than an HP Indigo?

Do you mean print on demand; like Blurb or Lulu?

If so, try A & I Imaging in Hollywood, or Edition One in San Francisco.
