This is a wild guess, so don't get too hung up on it. Have you done many many cleaning cycles? I remember reading about someone who needed to send their printer into Epson cause there is something in there which tracks the number of cleaning cycles performs, and when it's reservoir is believed to be filled with expelled ink the printer becomes inconsolable.
