It was a NMC CIS, directly from them when they still sold directly.

Running G4 colour pigs exclusively since new, probably three complete 4 oz 
bottle-sets worth of printing before failure.

I sealed the old CIS with tape, intending to re-use it, but decided not 
to.   When I get keen, I'll take it apart to see what happened.
