Digital BW, The Print

The problem is too short a print.   There is a 7 inch minimum length.
Unfortunately custom paper sizes in OS X are independent of which printer you
are using so OS X can't tell you the size is too small.  Illegal sizes default to 
Letter size which is what you got.
