Profile - to - Profile conversions....

Howard put up a message about profile-profile conversions. I was wondering how accurate this is when going from one color space to another. I know that the basis can be for different printing methods and therefore a 1:1 conversion may not be possible for the different gamuts. What about going from CIE/RGB or others to grayscale?
