A154 9-16/A3 out -> A150 CV-In
A155 CV-Out upper row -> A150 I/O In 1
A155 CV-Out lower row -> A150 I/O In 2
A150 O/I is the final CV-out

if you want the two trigger rows the same way you have to split the 
cable from A154 9-16/A3 out and feed it to the second switch in the 
A150. Alternatively you might hardwire the second CV-In on the A150 to 
the switchsocket of the first CV-in.
