hi-
although doepfer does make a very simple version of that, i must say 
they are very easy to make.  you can buy the jacks from an online 
electronics vendor and screw them into a panel to fit your modular or 
into, say, an "altoids" or sardine tin.  just wire all the jacks 
together in the appropriate tip/sleeve manner.

or you can get mono phone jack adaptors which go either way which you 
can install/remove from patch leads at your discretion.

these hover around a buck a piece at most online parts vendors.

since these kinds of cable converters/multiples are passive, i always 
find it a waste of panel space, and usually just have them floating 
nearby the synth.
