At first sight I could not see that the A-161 manual says that +5V appear at the outputs. In this case the manual would be wrong. The high state of the A-160 and A-161 outputs are close to +12V. It does not matter within the A-100 because any voltage beyond about +3V is treated as logical "high" (+5V as well as +12V) from any digital input (e.g. gate, clock, reset).
