If you are familiar with electronics that's possible but a lot of work. You have to interrupt the signal that leads from each potentiometer to the measuring input of the ADC-unit and replace it by a socket for the external CV. And you have to make sure that the external voltage is in the range 0...+5V (i.e. a protection circuit for each input). Negative voltages and voltages beyond +5V will destroy the electronics. If you only need the CV-to-Midi function (i.e. no potentiometers and the housing) I'd recommend the DIY/OEM products Pocket Electronic or USB64. If you want a module I'd recommend the A-192-1.
