I use a Minicraft drill and stand, similar to the Dremel units. It isn't very good - it's quite easy to break carbide drills. A friend of mine has a PCB dept. where he works and they have a rather old manual Excellon PCB drill. It looks as if it was originally *very* expensive and is *very* heavy. The drill comes up from underneath the PCB and a magnified image of the pad is shown on a screen.
