QTR-Quadtone RIP

I'm trying to make an ICC profile for with QTR-Create-ICC-RGB.app. Does anyone know the input file format? I've tried the following text file:

I used TextEdit on Mac OS 10.8.4 and also tried several different text encodings / line endings using Word.
