I\u02bbve been printing gray gamma 2.2 directly from Lightroom 4 to my Quad4880 printer, and seeing fairly good screen-to-print consistency, both with Cone profiles and with some Eboni profiles I made. I choose "printer manages color" in the print settings. (Of course, I might just have really low expectations.) Sounds like Photoshop CS6 is a disaster for B&W QTR printing.
