QTR-Quadtone RIP

As usual, I seem to answer my questions as soon as I post my questions...

It seems to have something to do with file size.  If I flatten the image, the preview shows.  It appears as if the maximum file size is around 600 MB

       I'm not sure what I'm doing
 wrong here.  I scanned some black and white images in RGBI
 mode in VueScan, converted to 16 bit greyscale.  When I go
 to QTR to print, most of the image previews display
 normally, but once in awhile I get a black preview.  If I
 convert those images to 8-bit, the image previews come up
 normally.  Why would some 16-bit greyscale previews display
 normally while others
