It looks like it's been removed from Northernlight Images. I've got a copy of the file if you wish to try it. Because of potential copyright laws, I probably cannot upload the file anywhere for general access.
However, that tool was extremely useful for fishing out the relevant Lab data from the Colormunki to linearise.
