there's also supposed to be a small tab inserted into the connector to make sure that the cards stay lined up with the edge connector vertically.  these are not glued in and are easy to lose.  make sure that your cards all have this tab still in place.  mine was missing a tab but there was an extra in an empty slot so i switched it over.

The problem is the so called edge connector, where 
the module slides into the connector of the backplane (I am talking about 
the white multipin connectors inside the unit). Two reasons: The fingers/tracks 
were tinned with a soldering iron. This wears out, you can see scars in the 
tinning. You have to re-tin it from time to time. Sometimes, if you transport 
the SDSV roughly, the edge connectors  break off the backplane. Then you 
have to re-solder them.
