Take a look at the Quantizer programs on my page http://www.ph.k12.in.us/~drew/noise/psim/ All you'd have to do is adjust the numbers in the VLOOKUP lists to correspond with the notes you want. The "Dual Quantizer" is set for 37 notes, but you can easily adjust that as well. (but if you want 121 note/10v range you'd be better off doing it with mathmatical functions instead of the VLOOKUP) For 1v/oct you just add 32 to go up a 1/2 step. For v/khz you'd have to experiment a little to get them just right, but once you figure out the math you're in business. I don't have a v/khz synth, so I'm not gonna be much help... I made a table of all the note numbers and their ADAC values on a laminated card, which I always keep handy.
