Are you sure you have pins 4 and 5 wired round the right way on the midi out
jack ? I've done this wrong myself before, so it's not impossible.

The CPU doesn't depend on any other chips to send the midi data, so that
can't be the problem. If there was a problem with the firmware, you would
get a checksum error on boot.
Does it sync to incoming midi clock OK ?
One small possibility - have you got the right value of resistor (220 ohms)
in the output line ?
