Is this in the most recent beta, b20 ?
There have been a few problems of that sort in previous betas, but I'm not
aware of any in the current build.
If it is in the current build, is there a sequence of events that can
reliably cause it to happen ?

I'm having the same problem. Whenever it happens i just shift to another part and then 
back again. It is pretty anoying though.

Move to quarantaine

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
