cgs90 phaser low volume at mix output

Hi,
I'm getting a really low volume at the mix output on the cgs90 phaser. The signals seem normal until after the 68k resistors on the mix section, which leads me to believe that the op amp should be providing some extra gain that it's not in my case. Everything appears to be soldered correctly, but I'm sure something went wrong somewhere. Any suggestions on what to look at next?
