You need to put in an email at the support section of our website. It looks like you've gone through most of the appropriate steps, so I assume they will swap out your Spyder, but I'll let them go through the support process first, rather then second guessing them from here..

Subject: [datacolor_group] green cast

I've recently bought spyder3 pro, and I i've calibrated monitor several times
now, with different set-ups, different sofware versions ( i first installed the
latest one from your site, then i uninstalled it and tried the one that was in
the CD bundled with spyder3. NOTHING seems to work I keep getting this strong
green cast.

My monitor has these:
-RBG sliders
-contrast
-brightness

I've tried it with telling the software:
-I have rgb sliders and backlight
-I have rgb sliders and contrast + brightness
-I have rgb sliders and back light contrast and brightness (this one with the
3.04)
-I have no rgb sliders, no contrast, no brightness and backlight only

With all these setups i've tried following preset targets:
2.2 6500
2.2 6500 LCD
2.2 Native

Nothing, and nothign seems to work and I keep getting this green cast. I know
some people have said that the get shocked after the calibration results, as
they've been used to blue tint, but this is just something else. The grays are
green-gray, the blues are lighter blues, and the color magenta is almost
inexistant ( i really have to pump up the colour balance to magenta in ps if i
want it ) and this calibration matches my prints even worse than the old one (
that was on uncalibrated monitor ).

Any ideas?
