If you can boot from floppy, have you tried formatting another floppy? Keep in mind, if you do not have DSDD (720Kb) floppy diskettes, only HD floppy Diskettes (1.44MB), that you must cover the hole (not the write protect hole, the other one). If the format fails, and you also cannot save banks, that sounds more like an hardware issue. Either the floppy drive is bad (bad head), or bad Floppy Controller IC. As to OS- if you have a Hard Disk, you must have an Hard Disk OS for the drive to work. What is the revision returned by going into Master> Special ? With a Hard Disk, you should be running either Emax HD OS 1.1, Emax HD SE OS 1.1 / Plus OS 1.0 (SE Only Emax).
