Do you have access to a USB hardware analyzer?  Have you sniffed the traffic
for improper traffic?

Keep in mind IAR recently released a Mass Storage example.  If I have time
I'll try your test code with it.

Move to quarantaine

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
