K.. I'm a bit late to the party here. : )

 How about for option C.. 

 3 pots: Rate, Phase, FM +/-

 1 Switch: Wave - 3 position (saw-square-tri) 

 1 Rate LED

 The 4 Jacks:
 a. Sync - works with Phase knob for use with other MOTM 390s :)
 b. Rectified ouput (has this been done b4?)
 c. FM input
 d. Output (saw-square-tri output)
