Actually, I like this circuit better, also from the Synth DIY list:

It allows the positive and negative set points to be adjusted, rather than
the fixed set points in Grant's circuit.  Due to the ease at which the
signal clips to the power supply rails, I'd probably add some limiting on
the output for the final circuit.  I did not see any slew rate problems at
high audio frequencies.

P1 = Positive Set Point
P2 = Positive Gain (+ or -)
P3 = DC Shift
P4 = Overall Gain (+ or -)
P5 = Negative Set Point
P6 = Negative Gain (+ or -)
