I'm going to have to counter the Cafe Press recommendation.  I don't 
think they do t-shirts well at ALL.  Their other merchandise is fine, 
but the t-shirts I did in the past faded substantially after a single 
washing.  They may have changed their shirt printing methodology, but I 
wouldn't risk it.

I have been working on t-shirts for some projects I am promoting, and 
VGKids (who I am not affiliated with in any way) did a great job - 
http://www.vgkids.com - and were reasonably priced.

As for a design, I would suggest something like:
