Are you sure the supply is DC? Seems strange that there is no volume control at all.

Yes, email the guy straight away with any and all your questions and
concerns.

Like said I suppose it could be a bad power supply. I'd think that
it's rated well enough @ 3amps, but could be putting out less. Is
the display well lit? Dim? If bright as star then maybe ok. But you
could always go to Wall-Mart, K-Mart, Radio Shack & pick up a power
supply or two (TODAY or the next). Maybe even one with switchable
voltages & a set of multi-plugs. Try to get to the closest rating
you can. I would prefer to have THE Yamaha one, just because.

I would also check with a 2nd pair of headphones, OR be SURE the
phones are ok. Have you tried the stereo L/R OUTS on the back of the
module yet with something? Check those! Maybe there IS a problem,
but only the headphone outs? I'd think that the headphone outs would
have their own output amp & the stereo L/R out should be line
voltage, so I'd try the L/R stereo outs on those powered speakers
you spoke of, or to an amp, stereo system, PA etc...
