You might try the Rolls MX28 Mini-Mix VI. It can handle three stereo inputs; it has a headphone jack; and it's very small. I don't know what your budget is, but you can get this one for $75. I've never heard a bad word about it. I use the Rolls headphone mixer, which is even cheaper. I know that zzounds carries the MX28, and other Rolls mixing options, and they have pretty good descriptions and pictures.
