That's not normal. Be sure you have not removed the reservoir plug. There are 2 holes in the top of the carts. One if for air to come in, the other for re-filling. On the MIS pre-filled carts I last saw, the refill hole was plugged by a rubber stopper that was flush with the top of the cart. So, removing this by accident is not likely.
It's probably the head.
If the viscosity was way too low, that might also do it, but that would only be in the case of a mixing mistake, not the standard Eb6 formula, which has been used for years in 1400's with far fewer problems than most other inksets.
