On more than one occasion I have had problems because the bottles were 
twisted slightly, suggesting that the tubes didn't have a clean throughput.
My bottles are in an awkward position (small apt), so I now make sure the 
caps are loose and clear of any twistage, and I screw the bottles onto the 
caps. Haven't had any problems since I've adopted this method.
