I've recently been reading "Photoshop Restoration and Retouching", by Katrin Eismann. There's not much that's specifically b&w, but I've learned a lot about different tools and approaches to tweaking photos. I picked it up because I was working on restoring some old family photos, but it really has a lot of techniques that have general application in image editing. Great step-by-illustrations.
