Missing 0% to 20% FSN

All,

Just printed Paul's 21stepNew.jpg file and I'm missing the 0% to 20% 
scale. It just prints white. I am using an Epson 1280 with the ARC 
T007 and ARC T009 FSN Carts, printing on Epson Achival Matte paper. I 
am using the Cone driver. Is there a fix for this? Normal? I 
appreciate any advise and suggestions.
