I've found nothing better than doing it manually using the clone stamp and
healing brush tools. I call it "mowing the lawn". Viewing the image at 100%
and filling the screen, i use keyboard shortcuts [(ctrl) page up/down] to
systematically go through the entire image starting at the top left corner,
moving left to right / top to bottom. ensures you don't miss anything and
maximal control in terms of not overdoing (or softening) the image.
provided you start out with a good clean scan, the process is not too bad.
takes me usually 1 - 1.5 hours per image. late at night, with some music, i
find it cathartic ;-)

Is there a plugin or procedure I could use to remove dust from a picture
based on a selection of the dust? I basically want to do the same thing
most scanners do in software based on the IR channel in an image. I use
vuescan with my FS4000US but the dust removal softens the image
noitcably, while if I scan the IR channel myself and adjust it in
photoshop I can get a good, clean selection that should enable me to
remove all the dust without any softening.
I've tried the dust & scratches filter, but it softens the image - I
want to maintain the grain structure. The polaroid dust and scratches
filter applied to the selection doesn't work that well either, it
doesn't remove enough dust, and when it does it also softens the grain
structure.
