The new layer dialog box appears if you use the menu or Alt (option?) click the new layer button in the layers palette. Once you select a blending mode, it enables the check box to fill the layer with that blending mode's neutral color. I too prefer to do burning and dodging on adjustment layers, but on the few occasions I use the burn/dodge tool, I set the "exposure" very low like 12 or 6 percent and make multiple passes to build up the effect I want. BTW, there is a nice technique described to open up shadow areas at imagingrevue.com (it's an excerpt from Katrin Eismann's book "Photoshop Restoration & Retouching").
